Thrive Together Program
Thrive Together is a short-term support program for youth ages 11–17 who are medically ready for discharge but need additional support before returning home or to the community. The program combines intensive care coordination and a safe, short-term residential option to help youth transition out of hospital settings, including Emergency Departments, CPEPs, and pediatric units. Referrals are accepted directly from hospitals.

The program focuses on a critical transition period and uses a time-limited, phase-based approach with clear goals at each stage. Supports adjust over time as youth and families gain stability, with strong collaboration across community providers and systems.

• Small caseloads to allow for hands-on, individualized coordination

• Ongoing team oversight through regular, structured meetings
Critical Time Intervention (CTI)
Youth and families are supported by a dedicated care team that provides hands-on coordination and frequent contact throughout
the transition process. Services typically last 6–9 months and focus on early engagement, problem-solving, and connecting families to lasting community supports.
The team works closely with caregivers, schools, providers, and other involved systems to support a safe and successful transition.
Transitional Residential Setting (TRS)
When needed, youth may stay in a home-like residential setting as an alternative to hospitalization. This option provides stabilization and respite while transition plans are put in place.
Key features include:
• Structured, supportive environment
• 24/7 supervision and monitoring
• Intensive crisis support
• Individual bedrooms (8 total)
• Nursing care
Stays are flexible and can be used at any point in the program, for up to 120 days total within the nine-month service period.
