The Care Manager for Health Homes serves as the primary point of contact with the enrolled child and their family to deliver the core services of Health Home; comprehensive care management, care coordination and health promotion, comprehensive transitional care, individual and family support, referral to community and social support services and the use of HIT to link services, as feasible and appropriate. The Care Manager provides ongoing advocacy and support to assure that the network of treatment providers involved with the enrolled child and their family delivers comprehensive integrated physical and behavioral health care that; recognizes the unique needs of the enrolled child and their family, is family-and-youth driven, and supports a system of care that builds upon the strength of the child and family.
Bachelor's Degree in Social Work, Psychology, Human Services or other related field with two years of relevant experience OR Master's Degree with one year of relevant experience or a Licensed Registered Nurse with one year of relevant experience required.
Valid NYS Driver's License
Provides outreach and engagement to enroll referred individuals into the Health Home program. Verifies Health Home Eligibility for referred individuals. Conducts the initial Child and Adolescent Needs and Strengths (CANS) assessment to evaluate the needs and strengths of the child and family. Coordinates and facilitates a Plan of Care.
