-The goal of the Bridges to Health (B2H) Waiver Program is to improve the health and well-being of children in foster care by providing support services in a child's community.
WSPs must be capable of providing services and making sevice decisions that take into account the needs and preferences of the child/medical consenter and caregiver. Many B2H Waiver services will be provided in the child's home, while other will be provided at other sites.
-Bachelor's Degree in Human Services or a related field and two years experience working with seriously emotionally disturbed children.
-Maintain documentation in the child's B2H case record that indicates progress, as well as any significant changes.
-WSPs are responsible for reporting cognitive, psychological, physical and/or behavioral changes to the HCI that may require intervention.
-WSPs are required to provide documentation of progress toward goal attainment from one IHP period to the next.
-WSP representatives must attend team meetiings for each child being served.
-WSPs must participate in the appropriate sharing of information related to delivery of B2H Waiver services.
-WSPs must offer comments for the Revised IHP, assist in tracing the extent to which children have met goals established in prior IHPs, and assess whether the goals have been met within the established time frames.
-WSPs arrange for translation services for potential or enrolled children and medical consenters with limited proficiency in English without undue hardship.
-WSP is repsonsible for providing all appropriate translation services nescessary to access services in the primary language of the child.
